"Celebrate Colorado's Chronic Care Model: Where We've Been...Where We're Going!" was brought to you by Colorado Clinical Guidelines Collaborative, Colorado Foundation for Medical Care, and the Colorado Department of Public Health and Environment. The conference was held on Friday, June 17, 2005 at the Radisson Hotel Denver Stapleton Plaza. A "Pre-Congress Awards Dinner" was held the evening of Thursday, June 16, 2005 for the participating clinics. Overall, the conference was well-attended (148 attendees) and was a huge success.
