Colorado Community Managed Care Network
Colorado Community Managed Care Network (CCMCN) is a network of Federally Qualified Health Centers serving 62 clinics in Colorado. Their patients are primarily medically underserved, low-income population. With planning grant funds, CCMCN assembled an advisory committee, conducted a survey of all 62 clinics, including key informant interviews and site visits to assess current tobacco cessation practices and willingness to implement comprehensive tobacco cessation interventions and systems change. The implementation phase includes implementing evidence-based cessation interventions into the care processes at five clinic sites as a pilot, developing an educational program for clinical and non-clinical staff, conducting a symposium to share lessons learned with all CCMCN clinics and developing a resources and strategies guide for sustainability. The program will expand to additional clinic sites with models developed at the five pilot clinic sites.
