New West Physicians
New West Physicians is a large primary care practice located in the Denver Metro area with 15 practice locations serving more than 250,000 patients. With planning grant funds, New West conducted a survey of provider and practice site perceptions of barriers and opportunities to improve tobacco cessation interventions in their system. The implementation phase includes incorporating tobacco cessation into their new electronic medical record system, integrating tobacco use status, readiness to quit and secondhand smoke exposure with vital signs assessment, training primary care providers and hospitalists, and providing routine feedback to practices.
Recent successes include training hospitalists and a nurse practitioner on inpatient smoking cessation interventions, and conducting patient surveys before and after the trainings to evaluate changes and provider behavior. Survey results showed significant improvements in the conduct of one-on-one inpatient counseling combined with offering cessation materials, resources, and pharmacotherapy prior to discharge from the hospital.
